Form results not going into access

I went through the wizrd to create the database. When the form is filled out
the confirmation page comes up but there's nothing in the access db. What am
I doing wrong?

I didn't code it. I chose send to database - form properties by right
clicking on the form. Then on the OPTIONS tab I selected CREATE DATABASE and
ADD CONNECTION, so there is a green check mark.

I'm just clueless as to why nothing is going to the DB.

Any suggestions? I've been at this for a couple of days now and am growing
weary.
