forms not working

I have been getting the following message when I submit the email form on my
site.Cannot run the FrontPage Server Extensions on this page:
http://www.summitmountainhomes.com/Feedback Form.htm

My web hosting service has reinstalled the extentions three times and I
still get the same message. They seem to think that the extensions are
corrupt. Any ideas?

A couple of things

1. It really isn't a good idea to use spaces in page names
or file names. Your form page URL has spaces. I would
remove these. If you need to separate the words, use the
underscore key. It is more effective.

2. You have your form file set to an .htm page in your
_private folder. If you want email and optional file, use
a text file in your _private folder.

If you don't have one there now, open a blank Notepad and
use save as. Name it something like form_results and save
it to the _private folder of the object web.

After republishing, the [FrontPage Save Results Component]
message should disappear and your form should work.

I took out the spaces and there is a text file in my _private folder. When I
open the form, open form properties, and click on the file results tab, the
file format is set up for HTML definition list. Should this be formatted
text? I am also getting the [FrontPage Save Results Component] message on
my form. Any other ideas?
