FP survey page

I put a FrontPage survey page into my website and it doesn't work. When you
hit submit a message page appears that says that it can't run Frontpage
extensions. The page is located at
http://www.caes.state.ct.us/aquaticplants/request_survey_lk.htm
I let Front Page put the survey in, the only thing I changed was the text so
I can't figure out why it isn't working.
The same thing goes for the hit counter on the home page

I let FrontPage put it in but it doesn't work.

I don't see an opening <form> tag on the page.
Also, the IUSR_Machinename must have write permissions on the _private
folder for the hit counter to advance.

Tom, thanks for replying to my post.
I believe that I have an opening form tag. Is this it <form method="POST"
action="../_vti_bin/shtml.dll/AquaticPlants/request_survey_lk.htm"
name="Suggest Lakes" onSubmit="" webbot-action="--WEBBOT-SELF--"> ?
I don't know what an IUSR_Machinename is or how to give it write permissions
on the _private folder (the only thing in the folder is index.htm.cnt which
when opened looks like FPCountFile 00000000000) . The FrontPage2003 book
made it sound like inserting a hit counter was simple. It said that all I
had to do was insert it and that all I would need it to be is a server-baed
web site stored on a server that has FrontPage Server Extensions, which my
server does.

Did you actually use http:// publish to the remote server, or FTP? You must
http publish to a site with the FP server extensions installed, or forms
won't work. Plus, it must likely will corrupt the extensions which your
host will have to repair or reinstall.

Regarding permissions on the hit counter, contact your host.
