getting cells to line up with headers (include page)

What is the best way to ensure your borders line up

ie; have a header added with an include page. Creating a
tabel with an image to the left and text in the right
cell.

each page will be made up of tables as well with
navigation links in the left side panel. So I need to
make sure the left side panel lines up with the include
page (headers) left side picture.

I'm guessing it's all in the size of the tables and
cells. pixels?

Do you feel this is a good way to create pages or should
I skip the include page and just put a header cell for
each page? or will I run into problems that way?

I assume your pages are set to center with a fixed table
width of 750 pixels or so.
It is a good idea to have the navigation, header and
footer as an include as it becomes
tedious to update these one by one even in small sites.
