Guest book doubles up form fields

I'm using Win xp. When I publish my guest book, the form
fields are coming up twice in my browser. When I "open"
the page in Front Page or "open web" in front page, the
form fields only appear once so I have no way of fixing
it. If you look at the page, you'll see what I mean.
