Header not showing on all pages

I copied and pasted the Header and Footer into the box and it looks good on
the view/header foot display box, but upon printing/print previewing it the
header only shows up on the first three pages (all on same tab/sheet) but the
footer shows up on all four pages. How do I fix this?
