how can i get rid of the text "formcheckbox" from my document

I was trying to add another option to a previously created form in the form
of a box that they could check if it applied.
When I went to tools forms and chose the check box it completely cahnged
all the existing check boxes from just a shadowed box that you could check
to showing text that says: "form check box" not only did it change the
document I was working in, but two other documents that have these options on
them.

Somehow you have probably toggled on the display of Field Codes. That can
be turned off via Tools>Options>View, or by pressing Alt+F9
