How do I add a new layout page to the slide master in ppt?

In addition to the existing layout options (content layout/text
layout/content & text layout) I'd like to create a completely different
layout that users can pick up. How do I do this in the slide master?

You can move, modify the size, change the fomatting of the placeholders given
to you, then save it as a presentation file. But you can't add any new
placeholders apart from the one given to you.

You will need to do so in the Slide Master.
Go to the Slide Master, change the formatting of the placeholders such as
the fill and font size.
Then, close Master View.

I'm sorry I don't understand, when I go to slide master it only gives me 2
pages which I can change (title slide and title and text slide). It doesn't
give me the title & 2 column text slide (NB I don't want it to have the same
attributes as the title & text slide)

If you change the attributes on the placeholders itself, it will also apply
to the one with title and 2 column text layout. If you do not want that to
happen, you will need to add an additional slide master (which is only
available in PowerPoint XP/2002 and 2003).

In Slide Master, click Insert > New Slide Master.
Modify the attributes in the placeholders.
Close Master View.
Now, you will notice that the placeholders still follow the attributes of
the first slide master.
Select the slides that you want them to follow the 2nd slide master (where
the attributes have been edited). Click Format > Slide Design.
Under Apply a design template, click on the 2nd slide master under "Used in
This Presentation". Is that what you want?

You can add slides to the template and they'll be automatically included when a
user creates a new presentation FROM that template. They can then copy and
edit the slide as needed.

You could also create example slides in another presentation and have the user
Insert, Slides, From File to bring them into the current presentation.
