How do i create a document wizard?

I'm working on a publication through microsoft word 2003. I want to be able
to have the same basic format for a document everytime i open it, but i need
a wizard at the beginnin that will ask for Date, Issue number, etc...then
will automatically plug that info into the template. Help!

To get the consistent format, of course, you need a template (.dot
file). You use File > New and select the template, and the new
document automatically inherits the formatting (styles, margins, etc.)
of the template.

The "wizard" will be what Word calls a "userform". Here are two
