How do I create a new envelope address template in Word 2003

When I use the address envelope function only a few of the relevant fields
appear in the Word document. How do I either create a new template or edit
the one that exists so that it pulls the first and last name on the first
line; company on the second, address on the third, and town, state and zip on
the last.
