how do i create a web based training document.

i would like to know how to create a document such as this

i have a bid form that i would like for client to complete in certain area's.

this is a word rtf file published to a webserver. To do forms like this,
simply turn on the forms toolbar by going to View - Toolbars- Forms.

from there, you can insert textboxes into documents and can be saved to a
webserver for access from the internet.
