HOW DO I CREATE FORM LETTER W/ PERMANENT LINES TO BE FILLED IN?

I am trying to create a form letter that I can go in each time and edit that
has lines that I write on. Ex: Name: ____________________________
Date: _____________________________
I want to be able to type that info on the lines without the line moving .
