How do I find a legal document to release my step daughter to me?

My step daughter with the approval of her mother, wants to live with me. She
is nearly 16 years of age, and I need to have a medical & general release
legal form signed by her and me, and then notorized. If you have any
suggestions please email me ASAP. Thanks.

A lawyer would have it, there are also legal software programs available,
but I don't know if any of them would have what you need. This newsgroup is
for the Access database program. You may want to ask your question in a
newsgroup about law subjects.
