How do I include a field such as "Dear [firstname]"?

I am trying to send a group email, and would like to include a field in the
email so that each recipient sees his or her first name in the email's
greeting. First Name does not appear in the "insert field" menu.

Are you using mailmerge?

Start by highlighting the Contacts you want to email merge to. Then Tools |
mailmerge | merge to email (bottom of pop up screen), write your subject
line. OK. Word fires up. Now you need to insert Merge Fields (NOT the
Insert Word Fields button) - it's the button to the left of the Word fields
one.
