How do I make and save a custom contact form

I am new to Outlook and want to know how I can make and save a custom
contact form. Is there a way to save the custom form as the default form?
If so, how do I save it as the default? I can figure out how to make a
form, but can't figure out what to save it as. Thanks in advance for the
help.
