How to restructure TOC

I need a TOC with the layout shownbelow (including the headers for each
column of info), but cannot find the right information to help me do it. I
have looked at TOC field switches and StyleRef, but could not find a method
that does not require a lot of user intervention (i.e., having to insert into
or modify each header that is pulled into the TOC). Can someone point me in
the right direction?

First page:
Table of Contents

Sec/Para Page
iii Title
1-1 Title 2
1-2 Title 6
.. . .

Subsequent pages:

Table of Contents - Continued

Sec/Para Page
3-5 Title 26
3-6 Title 36
T-1 Title 40
.. . .

Any help is appreciated.

I'm not clear what you want here, but it looks pretty much like what Word
provides by default. If headings are numbered in the text, the numbers will
be included in the TOC, and you can arrange the alignment of the numbers and
headings by modifying the format of the TOC styles. As for the "continued"
heading, you accomplish that by putting the TOC in a separate section with
"Different first page" enabled and the "continued" heading in the page
Header. The "Sec/Para" and "Page" headings can also be in the header, though
I don't think they're really required, since most people know how TOCs work.

Thanks for your help! I did manage to find the the TOC styles and modifed
them to give me the required layout. I found that I had to also modify the
TOC field code to put in a tab to get the page number to right-align (not
sure what I needed to do that because I thought Word did that automatically).
About the TOC header, I will try what you suggested. I thought Word did that
automatically somehow and was looking for a way to re-configure that
automation.

Ordinarily Word does set a right-aligned tab stop at the right margin when
you first create the TOC (it's generated dynamically, not part of the TOC
styles), but it can get lost if you omit numbering (on any level) or
sometimes if you just tinker with the style (I've never figured out what
causes it to go AWOL).

AFAIK, there is no way to create a TOC that automatically includes running
heads. An INDEX field can include headings for A, B, C, etc., but that's a
different type of thing.
