InfoPath 2007 and web browser enable

Hi, very appriciate your help on this issue:

I've created an IinfoPath form and I can publish it to SharePoint library as
web browser enabled form.

But after I added some event handling code using VB .Net, I'm not able to
publish it as web enabled. It giving me a message that form should be
approved by administrator. The first two options(Document Library and Site
Content Type) in publishing wizard are grayed out!

Thank you, Kaylan. I followed the instruction and tried to verify the form
template first using Central administration, but I'm getting couple of error
like below related to some of the variables inside my form:

An issue was found the following with variable
'Duty_Exception_Form.FormCode.FormID'. Member variables are not supported.
Use the FormState property to store data. Duty Exception Form.DLL
