inserting instead of attach

Recently when I try to attach a file it gets inserted into the email
instead. any ideas?

Change your format from Rich Text to Plain Text or HTML.

As far as I know, it has, but I wouldn't class myself as an Outlook "expert"
in all versions of Outlook.
