The only way I can figure out is to insert 12 pages, start the publication on page 3.
This will give you 3 and 4 back to back but then you will have two extra pages at the
end.
The only other way is to setup up pages 3 and 4 as a separate publication. In page
setup, select custom, landscape, 5.5 x 8.5. Insert a page, you will get 3 and 4 back
to back, but you will have two copies of each with this setup.

I guess i didn't give you quite enough info.

This is a merge catalog.

So to accomplish your suggestion i need to know how to copy pages 3 & 4
from the merge document and then i can delete them from the merge document.

does that make sence?

Open Publisher. Choose New from the File drop down menu, choose Blank
Publications, follow with Book Fold. When asked whether you want additional
pages inserted, YES. You will now have a four page document, advance to the
fourth page and from the Insert drop down menu choose Page, again respond to
add 4 pages to make a set.
When you actually print to paper, the page sets will allow you to make a
booklet.

Folded paper will always create four page sets. You could then use the empty
pages for some graphics, or organization's address, etc.

Many British newspapers have an odd number of pairs of pages (not a multiple
of four)

This can be accomplished by having a single A4 or Letter-sized page
somewhere within the A3 or Tabloid-sized booklet.

There is no way to automatically accomplish this in Publisher, you will have
to split the publication into two separate files, and manually renumber the
pages.
