Losing form when sending

Just wondering if anybody knows what the problem could be.
have .oft file on our intranet which when completed gets
sent to a user to process.
Recently there have been problems with fields not being
filled in corrrectly messages when they actually have been.

Now the user is receiving just a blank e-mail.

Any help would be much appreciated.
