Mail Merge Document - Adding a Check Box

I have a mail merge document that I want to change to a form. I want the end
user to be able to fill in 3 boxes plus choose a check box. However if I
lock the form then I can not change the mai merge reciepent's name. Is there
a way I can have both on mail merge and check box? Is there another way to
create a check box without using the form toolbar?
