Mapping into Outlook from Excel

Help:
I have gone to Tools/Forms/Design a Form and also Tools/Forms/Design This
Form.

Can I then import into the Newly Created Outlook fields from and Excel
spread sheet?

No. Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.slipstick.com/dev/customimport.htm.
