New Message Formatting

How Do I include a "from" field when sending a message? I use several accounts and I am constantly sending emails from the wrong account. I have (at least I think I have) changed the template but the from field still does not appear

Any help would be appreciated

Outlook version? Are these Exchange mailbox accounts or Internet accounts?
