Newbie question about forms

1. I've created a simple form for our sharepoint portal using FP2003. I
would like to have the form emailed to three different addresses. I've
gotten it to work with a single address but not multiple addresses.
2. I would also like to have a date chosen by selecting a calendar.
Highlight a date on the calendar and have it entered on the form.

Help needed solving this issue. Thanks in advance.

1. You'll have to use ASP to accomplish this.

2. You need JavaScript code to accomplish. Take a look at J-Bots Date Picker control

Wouldn't I have ASP with our server 2003?

You can send to multiple emails using a form chain ... A little labor
intensive, but not difficult.

You could also send your form to a forwarding address set to the multiples.
