What is the file extension of the document you received?

Have you tried opening it in Word?

If the form is indeed a text file with a *.txt file extension then the
check boxes you see can not be 'checked' in notepad, or any other
application for that matter, since the check box is a symbol character
(actually it's more likely it's an unknown symbol) rather than a real
check box control which will allow you to check/uncheck the box.

For example, in the Notepad hold Alt and press 0157 on the numeric pad
and a 'box' symbol will be inserted.

All you can do is copy/paste the file into a Word document and replace
the symbol characters with Word form field check boxes. However if you
do not know how to create a Word form then you may find it would be
faster to print the form and check the check boxes by hand or using a
typewriter.

I guess you could also replace the 'box' symbol with a 'checked box'
symbol (Insert/Symbol) from the Wingdings font in Word too. But again,
that could be time consuming.

Thanks to Beth & Echo for the reply on this question. It was realy doing my
head in!!!!
I have now solved my problems via another method....just deleting the
sections that were not applicable!!! If they can't provide a correctly
formatted form then they cannot expect it sent back as they wished it to be...

Once again, thanks for your time...
