Picture does not populate placeholder

When a picture is inserted into a placeholder, instead of populating the
placeholder it is apearing centered in the middle of the page

It is a generic "click icon to add content" placeholder that we are trying to
put a picture into, but instead of going into the placehlder the picture just
pops up in the middle of the page.
