Placing A Calendar in a form

I was wander how to place in a form. For example you a date question, then a
cell were a person can imput the date but right next to the cell is a pic/
link of a calendar that on they wave the curson on it it open up to callendar
were a person can select a specific date with month, year and day option????

question.
