Somehow the "Full screen" format for slide shows got turned "off". How do I
reset to get Full screen as the default (without having to right click and
select on every show)?

I can't see any way through the normal interface to change the default
setting on this.
Do you have language settings on set to A4
sizing?
Do you have any add-ins installed that run at startup?
Are you opening an existing presenation template that has the page
setup sizing set to a custom size (that's probably it).

What template do you normally use? Try opening the template directly, go to
File > Page Set Up, set it to On-Screen Show and save it as Design Template
(*.POT). Then close the file and start a new presentation your normal way. Is
it set to On-Screen Show?
