Print setup dialog

In File/Page setup there is a dialog box that defines the structure of
the print. The header in my system is defined at &T. What is &T?

I would like for my pages to have the title in the header and also
would like to have the modified date.

I have searched the KB and online documentation to no avail. Any help
would be appreciated. By the way Microsoft, this may be a good
article for the KB....
