Problem With Forms in FP2003

PLEASE, Ineed help. Created a form in FP. Gave me an option in properties to
send info to a file and or e-mail address. The file was
_private/form_results.csv. It would give me an error. Even though this file
exists, it wouldn't work. When I deleted the file from the properties window
and just left the e-mail address, the form worked, and that was fine. I don't
care about sending results to a file for storage, just sent to an e-mail. It
worked when I tested it about four times, then for some unknown reason, it
stopped. Now when you hit submit, it shows an error.

Your confirmation page is
"http://www.mylifesz.com/Show_Home/confirmation_page.html"
Change this to
"confirmation_page.html", and make sure that that page exists. The
confirmation page MUST be in the same website (in fact, the same
subweb) as the form.

Also, it is possible that the email address may need to be changed to
say, (e-mail address removed)
Some hosts require that addresses in emails from forms are in the same
domain as the website (though yours may be OK).
