Radio button on a custom form

I have created a form with 5 radio buttons. The Compose
side of the form works great. The problem is on the Read
page. No selection is appearing for the radio buttons

The radio buttons are bound to the same field value which
are type - Yes/No. I tried the format of Yes/No and
True/False and it did not make a difference.

Any ideas on what I am doing wrong??

This doesn't make sense: 5 radio buttons bound to a single field with only
two possible values (True and False). What am I missing?
