Is this Outlook installation patched to at least SP-1? If yes, turn on
diagnostic logging and post the .opm log for use to have a look at (even
better, post it to the Outlook group where you multi-posted.)
KB article 300479 will step you through the process to turn on diagnostic
logging.

| default account that does not do anything
| so that in case I forget to select an account, it does not go out
