Save All in My Tasks in PWA doesn't appear to work consistently

Several resources have encountered the following problem: They enter task
actuals, click save all, and close PWA. The next day or a few days later when
they're ready to update another day, the hours entered hours are GONE!

What could be causing this to happen?
