Jeff,

First off, the form relies on the FPSE installed on the
server.

Given this, having created your form, you may already have
a corresponding text file in your _private folder in your
web. If so, select this file e.g. form_results.txt as your
file destination. Set this to Formatted text and republish.

To read your text file, you would open your site live and
looking in the _private folder for your text file.

If the server has the FrontPage Server Extensions
installed:

1. Right-click the form and choose Form Properties.
2. Select Send To
3. Use the File Name box to specify the name and location
of the file.
4. Click Options to set additional options.

If the server lacks the FrontPage Server Extensions, the
host usually provides another program that can save form
results. Check the host's support pages for details and
sample code.
