Setting Up Custom Forms or Templates

I need to customize a document to email to various parties
to gather information that I will later input into another
software package. My intent is to setup a custom form or
template in Outlook and have the recipient email back to
me all of the information needed and then store it in
Outlook. I realize I can use a Word or Access Document as
an attachment, but I'd like to keep it within the Outlook
program. Please help,..........
