Sharepoint Issues - "unknown error"

I have found a couple of posts of people who have this same problem, but I
cannot find a solution. In the "Issues" area, when attempting to create or
edit an issue, when the Save Changes button is pressed, nothing visibly
happens (although the issue is saved correctly). The user will usually hit
the button again, making duplicates. This is annoying, but the bigger
problem is next (and hopefully related).

If I create a Custom List of issues (call it Dept. Issues) and try creating
a new issue or editing an issue, I get the following error:

"An unknown error occurred in updating the Microsoft Project Server
Database. Log on to Microsoft Project Web Access again and link the document
to tasks in the Tasks Page."

Of course, I'm not doing anything with documents when this occurs (and
Documents work fine).

STS and ProjectServer are on the same machine. I have installed the latest
MDAC (version 2.8), and rerun PSComPlus and ProxyCfg. I have done the
Server Health Settings and Synchronized Users (pretty much anything I could
find to do in this newsgroup). The server's hostname is listed as a Trusted
Site in IE.

Note: the Issue *WILL* be updated/created with the correct information as
you would expect, but you get that error pop-up as if there were a problem
somewhere. Nothing shows up in the Application Log.

Any clues?
