Template or permanent file for a form to be completed and reused

my form is complete but I do not know how to nor do I know the correct term
to make the document a permanent file that can be completed on line, and used
as an e-mail attachement or to be printed and then reused. The filled in
information can be deleted but the form itself cannot be changed and can be
used by others for ordering, etc.

The biggest problem with making a form with Publisher is that not a lot of
people have Publisher. Forms are best created with Word or any file can be
converted to a file with Acrobat Pro.

....to extend what JoAnn said...you could output your Publisher form as a
PDF, then if you have - or know someone who has Acrobat (the full version)
you could create a fillable form from the pdf. This is even better than
using a Word doc since your users would mess up the formatting when they
fill in the Word form.

Then you email the pdf form the user fills it out, saves it with their
changes and then emails it back to you.

have to have Adobe Reader v 7 or newer. (Can you tell I've been trying this
out with my own forms?)

I'm still using Acrobat 4 for this.

I use this method a lot for city permits...they provide permits on the web
so I took them all (the ones I use all the time) and converted them to
fillables...saves me a LOT of time when I hve to get permits for clients.

I had a friend who was using Publisher to create invoices & bids...yikes
what a mess! So I created a fillable pdf from his publisher forms...now
he's not mucking up the format AND he thinks I'm a bleepin' genius.

yA...I was saving up for STE of Adobe Suite 2.5...then 3 came out...now I'm
confused. I gotta buy it soon 'cause I have another ballet program to do in
the next few months.
