Templates question

I have a 2 page document that I need to make into template for our staff to
use. On page 1 (the front) there are 5 questions that need to be answered;
on the back there are 2 questions then a bunch of checkboxes. What I'd like
to do is to force the answer area for each question not to exceed 6 lines.
I've tried using text boxes, but they will force the following questions to
roll over to the next page, which they cannot do.

I'm stumped....any ideas? I have the document as I've created it worked up,
I could email it (no macros, virus free) if someone wants to see it.

Use a table with row height set to an Exact amount and put your text form
fields there.
