I have created a Word document and saved it as a .dot file. It is to be
filled in using Word but everytime I try to type something, the fields move,
underlines move and generally nothing is locked. We want to be able to type
to fill in the blanks without text or formatting shifting around. What am I
doing wrong?

You need to be using fields from the Forms toolbar and the form needs to be
locked after it is completed. Also, you need to use tables with bottom
borders so the lines don't move.

I can't be sure from your description whether or not you are using form
fields in a protected form. If not, you need to be; for more, see
http://word.mvps.org/FAQs/Customization/FillinTheBlanks.htm and especially
the forms tutorials by Dian Chapman that this article links to. To protect
the layout of your form, put your form fields in table cells (they can be
borderless or have a bottom border) with exact row height and column width.
