Once I’ve created a user form, besides opening the VBE, how does a users
access the form? Is it possible to place it on a cutomized toolbar?

Hi,

The user form must be called from a VBA procedure. A link to such a macro
can be put on a customizd toolbar.

Hi,

To call it
Nameoftheform.show
This transfers control to the form

In the code of the form when the input is received
Nameoftheform.hide

This transfers control to the procedure where you can use the data collected
After collecting all data you can
Unload Nameoftheform
