Using a published Custom Form

[SonomaEnterprises]

I design a form (select the contact form and do not change it) then publish
the form to the personal forms library.

Then I attempt to use that form (new-select form) I get a message that the
form could not be loaded....contact your admin....

I have cleared the cache(deleting the frmcache.dat and IPM folders), deleted
the form, recreated and re-published; but get the same message.

No matter what I cannot open a custom form.

I was running Outlook 2000 but upgraded to 2003 and get the same message,

Help, any ideas, I do not know what else to look at.....

I think you may try to create a new profile to see if that works.
BTW: can you tried to post the exactly error message.
