I am trying to create a survey at the end of the presentation that will be
distributed on discs - However, I would like the iinformation submitted in
the survey to go to an email vs. a directory. How can I make the Command
Button (Submit) do that?

Bill Foley has samples that allow you to email results from within PPT,
but the code is specific to the email client you use. If, for example,
you know that everyone is going to be using Outlook, his code will be
very helpful. If you can't control everyone else's email client, you
might have trouble. I think his sample is on his Web site, but I'm not
sure where. If you can't find it (and he doesn't chime in here himself),
you can at least find contact information for him at his Web site:
