when starting a new contact, it already contains info

when I open a new contact (Ctrl N) there is already information from a
previous contact already in the form. It is the same information on every
form. Is there a way to remove this duplication?

it sounds like you are using a custom form. Right click on the contacts
folder and choose properties. You'll have to option on that dialog to change
forms - see which form it's using and switch to the default form (IPM.Note).
if you need the custom form, you'll have to remove the content and
republish.
