While You Were Out FORM

Does anyone have the fix for accessing the form in Outlook 2000. The form I downloaded was for Outlook 98 but I can't seem to find the file to use it

What files were in the download once you installed it? Look for either ..pst or .oft.

I got it installed. it not what I would consider eye candy but it will work. Thanks for your assistance
