Does anyone know of a way to create a Word document so that only certain
fields are "editable". I want to send a form to someone and have them
edit specific fields ( name, address, etc ) and send it back to me or
someone else.

In Word, open the Help menu and select Microsoft Word Help. In the
Answer Wizard or Search type: how to create a form

That will show you how to create a form.
