I am attempting to create a form that will track customer information when
they call. The information includes date, time, issues or messages, customer
name, phone and address.

I would like to be able to key in the zip code and have it automatically
populate the city name in the city field of the form, but can't figure out
how to make this work. Can someone PLEASE help. Thanks in advance!

This would be easier if you use Access, or even Excel, instead of
Word. A database will offer you tracking capabilities you won't find
in Word.

Not that it can not be done in Word but you're talking about learning
VBA and ADO. In Excel it's a matter of implementing a few functions to
lookup the city from a zip code. In Access is as simple as creating a
query and using it as the record source for your form.

assistance by email can not be acknowledged.
