Word Template

I received a file that allows me to enter information, but
I an not able to change the basic format of the page. For
example: It has Name, Date, etc, but I cannot delete the
word Name or Date. I can only type in a space provided.

Is it a template? or s it just protected?

How would I go about creating something similiar?

What you are talking about is what Word calls an "online form." Check this
