You really have not supplied enough detail to be able too answer this
question.
You would not expect anything to appear in a document header or footer until
you type something in there.
The formatting of the text on that line is applied by the Header style.
Change the format of the Header style to change the formatting of the
headers.

After formating a document, when I click on the header and footer tab to
include document name and thread, only a blank straight line appears. The
