COM Add-in doesn't load when Excel launched by opening file

I am getting reports that my add-in does not load if Excel is launched
by opening an Excel file. For example, if Excel isn't running, and the
user double-clicks an .xls or .xlsx file in Explorer the add-in won't
load. But, the add-in loads fine when the Excel application is
launched via the Start menu or a Quick Launch shortcut.

This problem has been reported on Excel 2003 running on XP and Excel
2007 running on Vista. the earliest report goes back to the beginning
of 2009. I have not been able to replicate the problem in house.

Any ideas what might be causing this?
