You can use your Word document as a basis for the PDF form. However, you
will need to add the form fields using Adobe Acrobat, since form fields do
not convert when you create a PDF file from a Word file.

www.openoffice.org - will import your Word Document and print it as a pdf -
and it's free.

I accomplish this by having downloaded a free PDF Creator application.
use a couple different ones; each works equally well:
PDF995 -- http://www.pdf995.com/download.html
PDFCreator -- http://pdfcreator.com/
In either case follow link to FREE DOWNLOAD
This application, when installed, will appear as a printer in you
printer list. When you scan a document (or print a document from file
you select the PDFCreator (or PDF995) as though it were a printer. A
this end of the "conversion" process, you are given the option o
naming the new file and sending to a folder of choice where it will b
filed as a PDF file and from which it can be subsequently printed o
sent as an email attachment. Good luck
